Combination of in-clinic and work from home. You will be required to work in-clinic for one contiguous 3 hour block after 5pm on weekdays or a contiguous 3 hour block on weekends per week and reserve a 1 hour block in the morning per week to assess urgent cases for the clinic, both determined in conjunction with colleagues. No call. Typical appointment length is 20 minutes. Schedule is to be determined by the physician and the clinic will supply enough hours to reasonably supply patient care needs. Comfort with electronic medical records, chronic and acute disease management, immunizations, and excellent communication skills are required. Additional interest or training in geriatrics, palliation, pediatrics, minor procedures, and psychiatry are welcome. The facility uses Avaros - OSCAR based EMR with options for online booking and patient messaging, and the clinic offers billing support.  The incoming physician will be starting a new practice in the Income Stabilization program recruiting and managing patients in an area with varied patient backgrounds with a view to exit Income Stabilization when the practice has grown sufficiently. Payment structure will be via Income Stabilization which provides $17,407 per month for a 1 year period, after which the physician will enter a blended capitation FHO model. The clinic is LMIA ready (a core document needed for work permit), to welcome international medical graduates interested in this position. There are more than 20,000 patients without family doctor in the area, and more than 1,000 patients already on the clinic’s wait list just 8 weeks after opening, so building a roster will not take long.  Hours of work are normally 8-10 3-hour blocks which translate into 4-5 6-hour days including afterhours commitment as per Ontario government regulations. Hours are flexible and are set by the physician themselves. The physician can do more hours if they would prefer or less while still providing adequate care for their patients.   The clinic uses CHR EMR, and offers billing support. Payment structure is via FHO model for family practice, and fee-for-service in the Urgent Care Clinic. The split arrangement is 72.5/27.5, which will turn into 75/25 once the physician reaches a 2,000-patient roster. The Urgent Care Clinic is set up in a way that soon the cap for the physicians can be removed and there will be no penalty for rostered patients to be seen by other physicians within the urgent care clinic.
